Schedule: Full-Time and Part-Time Available
Pay: $30–$40 per hour (based on experience)

Destiny’s Caring Hands LLC is seeking a dedicated and experienced Registered Nurse (RN) to fulfill a hybrid role supporting our Adult Day Center and Integrated Health Care Coordination (IHCC) services. Key Responsibilities

IHCC – Hybrid Community-Based Care Coordination

  • Deliver professional nursing and social services based on physician orders and IHCP guidelines
  • Provide clinical care coordination and chronic disease education
  • Conduct regular outreach via phone, telehealth, and minimum of two in-person home/community visits per month (1 hour minimum)
  • Assist clients with navigating health systems, community resources, and transitions of care
  • Collaborate with interdisciplinary teams to develop individualized care plans
  • Maintain timely and accurate documentation in compliance with FSSA and Medicaid Waiver requirements
  • Support clients with medication management, adherence monitoring, and prevention planning
Adult Day Center – On-Site Clinical Oversight

  • Conduct health assessments, monitor vitals, and administer medications
  • Supervise care aides and ensure daily nursing protocols are followed
  • Respond to changes in participants’ conditions and communicate with physicians/families as needed
  • Participate in care team meetings and quality improvement planning
  • Ensure all nursing practices meet state, federal, and agency standards for adult day services
Required Qualifications (Per Indiana FSSA and Clinical Standards)

  • Active, unencumbered Registered Nurse (RN) license in the State of Indiana
  • CPR and First Aid certification
  • Minimum 1–2 years of nursing experience, preferably in home care, adult day services, case management, or community-based healthcare
  • Strong knowledge of Indiana Medicaid Waiver Programs including A&D, TBI, IHCC, and SFC
  • Experience with care planning, coordination, and documentation in Electronic Visit Verification (EVV) systems
  • Familiarity with HIPAA, infection control protocols, and FSSA care standards
  • Ability to work independently, manage time effectively, and support a diverse client base
