Position Overview
Destiny’s Caring Hands LLC is seeking a compassionate and experienced Licensed Social Worker (LSW) to join our team as a Hybrid Integrated Health Care Coordination (IHCC) Specialist. In this role, you will deliver professional social services and healthcare coordination to individuals enrolled in Indiana Medicaid Waiver programs.
You will play a vital role in ensuring that clients receive the services and support they need to live safely and independently in their homes and communities. This is a hybrid position, offering a mix of remote work and in-person client visits, with only two face-to-face visits per client per month required, in accordance with Indiana FSSA regulations.
Key Responsibilities
- Deliver comprehensive social work services and health care coordination based on physician orders and client needs
- Conduct biopsychosocial assessments and develop individualized service plans in collaboration with clients, families, and interdisciplinary teams
- Ensure continuity of care by facilitating communication among healthcare providers, case managers, and community organizations
- Provide education and support to clients and families navigating complex medical, emotional, and social challenges
- Monitor client progress, perform risk assessments, and make timely adjustments to care plans as needed
- Complete at least two face-to-face visits per client per month (one hour minimum) in compliance with IHCC/FSSA guidelines
- Maintain accurate, timely, and compliant documentation using Electronic Visit Verification (EVV) and case management systems
- Advocate for clients to ensure access to services, resources, and benefits that promote overall wellness and independence
Qualifications (Required by Indiana FSSA for IHCC Coordinators)
- Current Indiana license as a Social Worker (LSW)
- Master’s degree in Social Work (MSW) from an accredited program
- Minimum of two years of documented experience providing healthcare coordination or working with medically complex or waiver-eligible populations
- Knowledge of Indiana Medicaid Waiver programs (A&D, TBI, IHCC, SFC) preferred
- Familiarity with community-based resources and public health services
- Proficiency in electronic documentation and secure data handling
- Must pass background check, drug screening, and possess a valid driver's license with reliable transportation
Preferred Skills
- Strong communication and interpersonal skills
- Ability to build trust with clients and foster long-term engagement
- Experience working with elderly, disabled, or chronically ill populations
- Bilingual or multicultural experience is a plus
